Unless a court has stated otherwise (and a formal document can be provided to us), the parents listed on the birth certificate are the only people allowed to approve medical care being provided to a child. If a parent or LEGAL guardian isn’t bringing the child to their appointment permission is needed in writing from the parent prior to the child being seen. Please complete forms below to authorize an adult other than a legal guardian or parent to bring the patient to appointments.

Decisions will be made based on local weather conditions and communicated promptly through our website homepage.  Any scheduled appointments will be called in the event of closing.

Patients are advised to prioritize their safety and use their best judgment when traveling during inclement weather.  If you are unable to safely reach the office, please contact our office to reschedule your appointment.
